Order Form

Below is the link to our Instinct ORDER SPREADSHEET

A few things to note:

-The order spreadsheet must be filled out in SPREADSHEET FORMAT -- attached to an email. This means, not a screenshot, not a photo, not a handwritten page.
-You can use google sheets on your phone, computer or tablet, to open this spreadsheet, fill out, save, and send this spreadsheet.
-If you can't figure out how to use this spreadsheet, your order cannot be placed.

Also note, there are many columns besides name/number/size -- these are filled out if you are ordering multiple products. If you are ordering socks, belts, hats, hoodies, pants, etc., we need ALL the sizes for those items, for the corresponding player, listed.

IF YOU ARE ORDERING MULTIPLE ITEMS, PLEASE PUT THE NAME OF THE ITEMS AT THE TOP SO WE KNOW WHAT SIZE GOES WITH WHAT ITEM
